Position Overview:
The role involves delivering a range of patient care activities to support the Care Coach Connect Program, including clinical responsibilities in patients' residences and some administrative tasks.
Key Responsibilities:
- Execute all duties within the Certified Medical Assistant scope, including supervised procedures, injections, diagnostic testing, and phlebotomy.
- Coordinate healthcare screening results as per provider instructions and company protocols.
- Travel to patient homes and mobile units to facilitate service delivery in community settings.
- Conduct and finalize all quality program health screenings according to required metrics.
- Organize and accurately process documentation, ensuring all physicians receive patient results and quality forms.
- Maintain accurate and timely patient care documentation in medical records, adhering to company policies.
- Review medical information and test results before patient discharge to ensure comprehensive data capture.
- Manage inventory of supplies, ensuring availability and accuracy of stock, while addressing discrepancies.
- Promptly present abnormal results to the provider to address any urgent conditions.
- Prepare clinical team charts for post-visit processing, including faxing and filing.
- Ensure all results are compiled and chart notes entered prior to patient visits, including necessary records.
- Document follow-up appointment dispositions in the clinical system.
- Verify completion of post-visit recommendations, such as prescriptions and referrals.
- Uphold patient confidentiality and manage the proper release of information.
- Maintain certifications and adhere to quality control standards.
- Foster a positive work environment and assist team members as needed.
- Perform additional related duties as assigned.
- Operate diagnostic equipment and assist with basic medical procedures under supervision.
- Facilitate timely entry and follow-up of referrals.
- Support the development of Plan of Care documentation for review by the assigned clinician.
Qualifications:
- High school diploma or GED equivalent.
- Current Medical Assistant certification or registration required, or must obtain within 90 days of employment.
- Current CPR certification or must obtain within 30 days of employment.
- Basic computer skills and knowledge of medical terminology.
- Familiarity with ICD-10 and CPT coding.
- Ability to remain calm and effective in emergency situations.
- Willingness to travel frequently within the assigned market.
- Flexibility and adaptability to change.
- Reliable transportation for daily travel.
Preferred Qualifications:
- Graduation from an accredited Medical Assistant program.
- 3+ years of experience as a Medical Assistant or a combination of 5+ years of non-certified and certified experience.
- 1+ years of relevant experience in a medical environment.
- Bilingual proficiency (English/Spanish) is a plus.
- Strong communication and customer service skills.
Physical & Mental Requirements:
- Ability to lift up to 50 pounds.
- Capability to push or pull heavy objects.
- Ability to stand for extended periods.
- Proficient use of fine motor skills for equipment operation.
- Ability to comprehend instructions both verbally and in writing.
- Logical reasoning skills for problem-solving.
- Occasional exposure to communicable diseases or bodily fluids.
