Senior Clinical Care Coordinator Role

We are seeking a highly skilled Senior Clinical Care Coordinator to join Sevita, a leading provider of home and community-based specialized health care. As part of our team, you will play a vital role in delivering exceptional care services to individuals with brain injuries, intellectual and developmental disabilities, substance use disorders, and emotional or behavioral challenges.

About the Position

This is an exciting opportunity to leverage your expertise as a Licensed Practical Nurse (LPN) to provide high-quality nursing services, training, and support to individuals served. You will be responsible for monitoring their health conditions, administering medications, providing documentation, and overseeing staff members who administer medications.

Key Responsibilities
  • Provide comprehensive healthcare services, including medication administration, wound care, and personal care.
  • Conduct thorough assessments to identify individual needs and develop personalized care plans.
  • Train staff members on universal precautions, infection control, and medication administration to ensure high standards of care.
  • Serve as a liaison and advocate with medical professionals, external agencies, and families to coordinate services and ensure consistent treatment.
  • Collaborate with interdisciplinary teams to address complex medical and behavioral needs of individuals served.
Requirements and Qualifications
  • Associate's Degree in Nursing or related field, or state certificate
  • One year of experience working with population served in a nursing capacity
  • Current state LPN licensure for state of residence
  • Current driver's license, car registration, and auto insurance
  • Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
  • Excellent communication and interpersonal skills to effectively collaborate with staff, individuals served, and families.
