Position Summary:

  1. Facilitates patient care coordination, including the scheduling of appointments in alignment with physician availability while adhering to established policies and procedures.
  2. Coordinates outpatient testing and procedural appointments as necessary.
  3. Handles patient registration tasks, including the accurate entry and verification of demographic and insurance details, as well as obtaining necessary referrals and authorizations.
  4. Manages the creation and coordination of outgoing referrals.
  5. Provides patients with Facility Fee Disclosures, estimates of charges, and relevant educational information.
  6. Prepares after-visit summaries, notifies patients of upcoming appointments, schedule changes, co-payments, and essential insurance information.
  7. Offers information to patients, family members, and referral sources regarding hospital policies, available programs, navigation assistance, and parking options.
  8. Demonstrates the ability to multitask effectively in a fast-paced environment while maintaining professionalism and organization.
  9. Assists in tracking and ordering office supplies as needed.
  10. Utilizes a multi-line phone system and software in a call center environment to manage patient care coordination calls, including scheduling and canceling appointments, verifying demographic information, and documenting interactions within the EMR system.
  11. Participates in regulatory and accreditation surveys as required.
  12. Performs routine clerical tasks such as photocopying, filing, faxing, and maintaining communication between clinicians, administrative staff, and patients.
Qualifications:

Education and Experience:

  1. High School Diploma or equivalent (GED) is required.
  2. Experience in a healthcare environment is preferred but not mandatory.
Knowledge, Skills, and Abilities:
  1. Understanding of various insurance coverages, including Medicaid and HMOs.
  2. Strong verbal, written, and interpersonal communication skills to effectively convey hospital policies and procedures.
  3. Ability to complete intake forms and respond promptly to instructions.
  4. Effective listening skills to accurately capture and document patient information.
  5. Proficient in using computers to enter and retrieve patient information.
  6. Capability to establish and maintain courteous relationships that promote quality customer service.
  7. Resourceful problem-solving skills for handling inquiries and scheduling issues.
  8. Ability to work independently and manage stress effectively.
  9. Strong organizational skills to prioritize and manage multiple tasks.
  10. Adherence to hospital and departmental policies, including patient privacy regulations under HIPAA.
