POSITION SUMMARY
The Clinical Nursing Supervisor is responsible for overseeing the management and leadership of a designated nursing unit. This role involves evaluating available resources and delegating tasks effectively to ensure optimal care outcomes. The Supervisor also directs and executes the nursing process, which encompasses systematic data collection, assessment, care plan development, implementation, and revision.
ESSENTIAL RESPONSIBILITIES AND DUTIESRESPONSIBILITY: To Facilitate the Comprehensive Care Planning Process- Systematically gathers data using the MDS about residents through interviews with residents and families, observations, discussions with physicians, and reviews of medical records.
- Evaluates residents and applies expertise to develop the most effective therapeutic care plans, utilizing nursing diagnoses as appropriate.
- Collaborates with the interdisciplinary team to assist in the formulation of care plans using the CAAS.
- Implements care plans by assigning responsibilities, establishing communication systems, and monitoring outcomes.
- Participates in care conferences to address the needs of residents and their families.
- Monitors changes in resident conditions and updates care plans accordingly.
- Supervises the completion of functional care plans by other nursing staff.
- Maintains knowledge of and adheres to the residents' Bill of Rights.
- Utilizes standards of care and specific policies to perform nursing procedures for residents.
- Encourages autonomous decision-making among residents whenever feasible.
- Promotes the Continuous Quality Improvement program of the facility.
- Contributes to recommendations and changes in nursing administration that impact resident care.
- Engages in ongoing education and reviews literature to enhance skills in gerontological nursing.
- Performs lab draws or delegates this task as appropriate.
- Clearly communicates expectations and delegates appropriate duties to nursing staff.
- Follows up on assigned tasks to ensure completion.
- Conducts performance evaluations as assigned by the Director of Nursing.
- Assesses the knowledge and skills of nursing personnel to ensure safe practice.
- Addresses unprofessional behavior among staff as necessary.
- Prioritizes and adjusts work assignments based on staff availability and resident care needs.
- Takes necessary actions to maintain appropriate staffing levels, which may include working as a staff nurse.
- Understands and implements emergency plans for various situations.
- Works designated weekends and is available for on-call duties as required.
- Completes documentation in accordance with health information standards and facility policies, particularly for residents on Medicare Part A.
- Systematically collects and communicates resident assessment data through reports and ensures accurate documentation in medical records.
- Documents data related to admissions and discharges.
- Facilitates physician orders and ensures accurate follow-up procedures.
- Initiates reports and documents nursing orders as required.
- Provides updates to nursing assistants and other staff regarding daily resident care needs and significant changes in condition.
- Follows up on recommendations made by dietitians.
- Administers medications accurately in accordance with policy, including hand hygiene and the five rights of medication administration.
- Aims to maintain a medication error rate of less than 5%.
- Signs out medications and treatments according to established protocols.
- Reviews medication and treatment records monthly.
- Follows proper procedures for label changes.
- Adheres to emergency kit protocols.
- Complies with controlled substance policies.
- Coordinates the destruction of controlled medications with a pharmacist and non-legend drugs with the Director of Nursing.
FACILITY COMPLIANCE
- Fosters a positive workplace focused on exceptional customer service and inclusivity.
- Maintains current licensure and keeps it updated.
- Participates in continuing education and in-service training relevant to the department.
- Attends all staff education sessions related to safety, infection control, and disease processes.
- Engages in educational programs on Continuous Quality Improvement and customer satisfaction.
- Stays informed about regulations from the Minnesota Department of Health.
- Identifies areas for improvement through surveys and communicates findings at quality assurance meetings.
- Demonstrates flexibility to work evenings and weekends as directed.
- Must possess a current Registered Nurse license in the State of Minnesota.
- Must have completed or be pursuing certification in Cardiopulmonary Resuscitation and emergency care for healthcare providers.
- Must demonstrate effective verbal and written communication skills in directing resident care.
- Must be capable of performing essential job functions with or without reasonable accommodation.
- Must pass a criminal background check as per Minnesota Department of Human Services guidelines.
- Experience in healthcare settings, skilled nursing facilities, or assisted living communities.
- Bachelor of Science in Nursing degree.
- Ability to manage multiple tasks effectively and adapt to sudden changes.
- Resilience in receiving constructive feedback and making necessary adjustments.
- Personable demeanor to build trust with residents, staff, and families.
