Job Responsibilities
- Provide personalized care and support to clients in their homes, promoting independence and well-being
- Develop and implement customized care plans to meet clients' unique needs and goals
- Collaborate with healthcare professionals to ensure comprehensive care delivery and coordinate medical appointments
- Monitor and document client progress, adjusting care plans as needed to achieve optimal outcomes
- Participate in ongoing education and training to enhance knowledge and skills in areas such as wound care, fall prevention, and medication management
Requirements
- STNA certification or HHA training preferred; CPR certification required
- Minimum 1 year experience in home health care or a related field; experience with elderly or disabled populations a plus
- Excellent communication and interpersonal skills to build trust with clients and their families
- Ability to work independently and as part of a team to achieve goals and objectives
- Physical stamina to perform duties related to personal care and mobility
Salary and Benefits
$11.00 - $15.00 per hour, depending on experience and qualifications. Benefits package includes paid time off, opportunities for professional growth and development, and a supportive work environment.
