Nurse Case Manager Role at a Leading Medical Center

We are seeking a highly skilled Registered Nurse Case Manager to join our team at St. David's Round Rock Medical Center.
As a Registered Nurse Case Manager, you will play a critical role in promoting patient-centered care by coordinating the plan of care for patients, managing length of stay, ensuring appropriate resource management, and developing safe and effective discharge plans in collaboration with the multidisciplinary team.
Key Responsibilities:- Provide case management services for both inpatient and observation patients as assigned
- Identify patients at risk for adverse outcomes during transitions between care levels or settings
- Perform comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as their resources and available support systems
- Reassess patients' clinical conditions as needed, considering readmission status or risk and developing strategies to mitigate these risks
- Coordinate the plan of care and drive discharge planning by collaborating with the healthcare team, particularly with patients' physicians, to facilitate successful care transitions
You will work closely with Social Services to ensure that post-acute medical needs and level of care are appropriate.
RequirementsTo be considered for this role, you must have:
- 2+ years' experience in case management or 3+ years' experience in clinical nursing
- InterQual experience is preferred
Education and Licenses:
- Associate Degree in Nursing or Nursing Diploma
- Bachelor's Degree in Nursing is preferred
- Current licensure as a Registered Nurse in the state(s) of practice according to law and regulation
- Certification in Case Management is preferred
We offer a competitive salary range of $75,000 - $90,000 per year, depending on experience.
