We are seeking a skilled Registered Nurse to join our team as a Case Manager at St. David's South Austin Medical Center.

Job Summary:

This role is responsible for coordinating patient care, managing length of stay, and ensuring appropriate resource utilization while promoting quality outcomes and efficient throughput.

  • The ideal candidate will have 2+ years' experience in case management or 3+ years' experience in clinical nursing.
  • InterQual experience is preferred, but not required.
Responsibilities:

This position involves the following key responsibilities:

  1. Providing case management services for both inpatient and observation patients
  2. Identifying patients at risk for adverse outcomes during transition from one level of care/setting to another
  3. Performing comprehensive assessments of psychosocial, medical, and discharge needs of patients/families, including assessment of available resources
  4. Reassessing patients' clinical conditions as indicated, considering readmission status or risk of readmission, and developing strategies to mitigate these risks
  5. Coodinating plans of care and driving discharge plans through collaboration with multidisciplinary healthcare teams and patient physicians to facilitate successful care transitions
