Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Williams Bay Health Services. As a member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the well-being of our residents.

Job Summary:

Under the supervision of the RN Unit Manager or DON, the LPN/LVN will perform as a licensed caregiver and assume responsibility and accountability for assigned residents/patients for a shift of duty. Nursing care will be provided through coordination, implementation, and evaluation of the resident's/patient's plan of care.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Develop and implement individualized care plans to meet the unique needs of each resident/patient.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Supervise and educate nursing assistants to ensure they are providing high-quality care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Current licensure as an LPN in the state of employment.
  • Current CPR certification.
  • Ability to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, medication cart, etc.
  • Ability to calculate drug dosage.
  • Meets all health requirements imposed by law.
  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff.
  • Able to read, write, speak, and understand English.
