Role Summary

As a Hospice Medical Records Nurse with Chapters Health, you will play a vital role in providing outstanding patient care and customer service in our communities. Under the supervision of an RN, you will be responsible for providing nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.

Key Responsibilities

  • Provide nursing tasks to patients and families, including continuous care, dressing changes, and catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
  • Demonstrate clinical skills regarding application of Company policies and procedures, such as medication administration, symptom management, and patient/family relationships.
  • Document patient information per policy and procedure in medical records.
  • Participate in Quality Assessment and Performance Improvement activities as appropriate for the position.

Requirements

  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • Mobile Driver - Valid driver's license and automobile insurance per Company policy.
  • Ability to communicate (written and verbal) in English.
  • Flexible schedule to meet the needs of patients.
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to perform the essential functions and physical requirements of the job with or without reasonable accommodation.
