Job Title: Staff - Float Physical Therapist

Advocate Aurora Health is seeking a per diem / prn Float Physical Therapist for a per diem / prn job in Hartford, Wisconsin.

Job Summary:

We are seeking a skilled and compassionate Physical Therapist to join our team as a Float Physical Therapist. As a Float Physical Therapist, you will have the opportunity to work with a diverse range of patients and provide high-quality care in a dynamic and supportive environment.

Responsibilities:
  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
  • Establish a plan of care and implement individual treatment based on evaluation results and functional goals.
  • Monitor and evaluate outcomes and appropriately adjust the individualized treatment plan based on the patient's response to treatment and provide status updates to physicians.
  • Identify factors affecting patient's physical and psychosocial comfort and attend to these needs.
  • Plan for and complete patient discharge including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
  • Complete all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate, to record patient care and progress of each patient assigned as well as daily statistics, billing, and scheduling.
  • Apply insurance guidelines regarding authorization and number of visits.
  • Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly.
  • Perform various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
  • Demonstrate skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist.
  • Demonstrate knowledge of indications and contraindications of modalities and exercise.
  • Participate in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care.
  • Manage assigned caseload, informing site leadership of coverage needs and ability to cover; assist with coverage, based on department needs and regional needs.
  • Pursue professional growth through participation in continuing education courses, department inservices, and reading professional literature.
  • Orient new staff to clinical and department policies as requested.
  • Participate in the evaluation process of co-workers by providing verbal or written feedback as requested.
Requirements:
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.
  • Physical Therapist license issued by the state of Wisconsin.
Preferred Qualifications:
  • Bachelor's Degree in Physical Therapy.
  • Intermediate computer skills.
  • Excellent communication and interpersonal skills.
  • Demonstrated organization, decision-making, and problem-solving skills.
Work Environment:

This position requires the ability to sit, stand, walk, lift, and squat throughout the workday. The position also requires the ability to lift up to 50 lbs. from floor to waist, lift up to 10 lbs. from waist to overhead, and lift and carry up to 40 lbs. at waist height a reasonable distance. The position also requires the ability to push/pull with 30 lbs. of force, perform a sliding transfer of 150 lbs. with a second person present, and position requires repetitive use of hands: simple grasping - 5-15 lbs. - 20% of the workday, pushing/pulling - up to 75-100 lbs. - 10% of the day, fine manipulation of hands during massage and joint mobilization.

This position is exposed to electrical and chemical hazards as well as blood and body fluids; therefore must have ability to wear protective clothing as needed. The position may require travel; therefore may be exposed to road and weather hazards.
