About the Role

We are seeking a skilled and compassionate Hospice Certified Nursing Assistant to join our team at Gentiva Health Services. As a key member of our interdisciplinary care team, you will provide high-quality patient-centered care to individuals in need of palliative and hospice services.

Job Summary

The successful candidate will possess excellent communication skills, a caring attitude, and a strong commitment to delivering exceptional patient care. This is a safety-sensitive position that requires a valid driver's license and current CPR certification.

Responsibilities
  • Provide basic care and support to patients in their homes or other settings, including assistance with daily living activities, personal hygiene, and medication management.
  • Assist patients with light meals, ambulation, and transfer exercises, as well as promote mental alertness through engaging activities.
  • Conduct routine vital sign assessments, monitor patient condition, and report any changes to the Registered Nurse and other members of the care team.
  • Maintain accurate documentation of patient care and outcomes, including visit itineraries and quality improvement initiatives.
Requirements

To be eligible for this role, applicants must:

  • Hold a valid Certified Nursing Assistant (CNA) license.
  • Demonstrate six months of patient care experience in hospice, home health, ICU, geriatric, or oncology settings.
  • Possess a current driver's license, automobile insurance, and CPR certification.
