LPN - HEARTest Yard Infants Care Specialist

BAYADA Home Health Care is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our HEARTest Yard program. As a member of our team, you will provide high-quality, patient-centered care to infants with congenital heart disease and their families.
Program Overview
The HEARTest Yard program is a unique partnership between BAYADA Home Health Care and the Greg Olsen Foundation, providing in-home private nursing care to families of babies born with congenital heart disease. Our goal is to ease the transition from the hospital to home and support the long-term health of these children.
Responsibilities
- Provide one-on-one care to infants with congenital heart disease
- Collaborate with the healthcare team to develop and implement individualized care plans
- Assist with daily living activities, such as bathing, dressing, and feeding
- Monitor and report any changes in the infant's condition to the healthcare team

Requirements
- Current, valid LPN license
