About the Role

We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team at Thrive Skilled Pediatric Care LLC. As a key member of our care team, you will provide high-quality clinical home care to medically fragile children, enabling them to grow and flourish to their full potential.

Responsibilities
  • Provide one-to-one care to pediatric clients in their homes, promoting their physical, emotional, and developmental well-being.
  • Develop and implement individualized care plans, working collaboratively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Administer medications, perform medical procedures, and provide wound care, as needed.
  • Monitor clients' conditions, reporting any changes or concerns to the care team and healthcare providers.
  • Collaborate with the care team to ensure continuity of care, sharing knowledge and expertise to enhance client outcomes.
Requirements
  • Current nursing license and current CPR certification.
  • Experience working with pediatric clients, preferably in a home health care setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and healthcare professionals.
  • Ability to work independently, with minimal supervision, and as part of a team.
  • Flexibility to work varied schedules, including weekdays, evenings, and weekends.
