Responsibilities- Coordinate the care of residents, including developing and implementing individualized care plans
- Collaborate with healthcare teams to ensure high-quality care and services
- Conduct regular assessments and evaluations to identify resident needs and preferences
- Develop and maintain relationships with residents, families, and healthcare providers
- Ensure compliance with regulatory requirements and company policies
- RAC-CT Certification preferred
- Valid RN or LPN/LVN licensure issued by the State Board of Nursing
- Valid CPR certification
- Knowledge of nursing and medical practices and procedures, as well as state and federal regulations
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
