We are seeking a skilled RN/LPN MDS Coordinator to join our team at Plainfield Healthcare. As a key member of our care team, you will be responsible for coordinating the care of our residents, ensuring their needs are met, and promoting a positive and supportive environment.

Responsibilities
  • Coordinate the care of residents, including developing and implementing individualized care plans
  • Collaborate with healthcare teams to ensure high-quality care and services
  • Conduct regular assessments and evaluations to identify resident needs and preferences
  • Develop and maintain relationships with residents, families, and healthcare providers
  • Ensure compliance with regulatory requirements and company policies
Requirements
  • RAC-CT Certification preferred
  • Valid RN or LPN/LVN licensure issued by the State Board of Nursing
  • Valid CPR certification
  • Knowledge of nursing and medical practices and procedures, as well as state and federal regulations
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
