As a Registered Nurse with IntelyCare, you will be responsible for providing high-quality care to patients in post-acute facilities, working collaboratively with physicians and other staff to develop and implement effective care plans. Your duties will include:

• Providing direct patient care, including administering medications and treatments.
• Developing and implementing individualized care plans, in collaboration with patients and their families.
• Monitoring patient progress and adjusting care plans as needed.
• Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination.

To be successful as a Registered Nurse with IntelyCare, you will need:

• A current Pennsylvania state license to practice as a Registered Nurse (RN).
• Graduation from an approved nursing program.
• At least 1 year of experience as a Registered Nurse or in a similar role.
• Excellent organizational and leadership skills, with the ability to work independently and as part of a team.
