Key Responsibilities:

  • Develop and maintain relationships with community partners, hospitals, and facilities to drive growth and development targets.
  • Facilitate patient admission process and discharge from hospital to home, ensuring seamless transitions and high-quality care.
  • Assess referred patients' clinical needs and disease status, collaborating with physicians and other healthcare professionals as needed.
  • Communicate effectively with patients, families, and caregivers to understand their needs and provide tailored support.
  • Collaborate with the Supervisor to develop and implement strategies for promoting Compassus services and benefits.
  • Maintain accurate records and documentation of patient interactions, progress, and outcomes.
  • Participate in educational programs and staff meetings to stay up-to-date on industry developments and best practices.

Requirements:

  • RN License required; Bachelor's degree in Nursing highly preferred.
  • Two to three years of related experience required, preferably in hospital or long-term care settings.
  • Hospital and/or long-term care clinical experience highly valued.
  • Experience in healthcare marketing an asset.

Estimated Salary: $95,000 - $130,000 per year
