Job Title: Home Care Registered Nurse

At Children's National Hospital, we are seeking a highly skilled and compassionate Home Care Registered Nurse to join our team. As a Home Care Registered Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their homes, promoting independence, and improving health outcomes.

Key Responsibilities:
  • Conduct thorough assessments of patients' needs and develop individualized care plans to address their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal health outcomes.
  • Document patient progress, identify areas for improvement, and implement changes to care plans as needed.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
Requirements:
  • Current RN licensure in the District of Columbia or Maryland.
  • Minimum of 1 year of experience in home health care or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Basic computer skills and proficiency in electronic medical records.
