Work Schedule

This is a full-time salaried position with a Monday to Friday schedule, ensuring no weekends, evenings, on-call duties, or holiday shifts.

Travel Requirements

Daily local travel to patients' residences is necessary, with all mileage reimbursed. We provide all necessary equipment, medical supplies, and training.

Qualifications

  • Graduation from an accredited nurse practitioner or physician assistant program.
  • A clinical background in adult, family, geriatric, podiatry, or vascular specialties.
  • Minimum of one year of experience as an advanced practice provider.
  • Board Certification/Preparation through AANP, ANCC, or NCCPA.
  • Experience in wound care is advantageous.
  • Nurse Practitioner national certification as ANP, FNP, or GNP is required or must be obtained.
  • Possession of a current, unrestricted license in the state of practice or the ability to obtain one.
  • Capability to secure and maintain State DEA licensure / Prescriptive Authority where applicable.
  • Familiarity with Microsoft Office and CRM systems is preferred.

Practice Responsibilities

  • Deliver advanced wound care to patients in their home environment.
  • Conduct thorough health assessments for wound care patients utilizing advanced practice skills.
  • Formulate and execute individualized care plans, ensuring coordination of care between visits.
  • Perform comprehensive patient evaluations, focusing on factors that elevate the risk of wound development.
  • Administer advanced treatments, including surgical debridement, UltraMIST therapy, and skin substitutes.
  • Manage medications by prescribing necessary treatments and providing clear usage instructions.
  • Educate nursing staff and maintain effective communication with primary care providers, families, and clinical teams as needed.
  • Contribute to the development of efficient practice management policies that enhance patient access to care.
  • Exhibit the ability to operate at the highest level within the scope of practice.
  • Demonstrate expertise in patient education and proactive disease management.
  • Adapt care approaches to meet the developmental needs of the patient population served.
  • Show a strong commitment to delivering exemplary patient care.
