Key Responsibilities:
The Case Manager is accountable for coordinating healthcare decisions through a systematic approach to ensure treatment plans that enhance quality and outcomes. This includes:
- Coordination of care across the continuum
- Promotion of cost-effective care within designated timeframes
- Assurance of payment for hospital-based services meeting patient-related utilization management criteria
- Implementation of safe and effective discharge plans.

The Case Manager will assess the social work needs of patients and provide necessary social work interventions as part of the discharge planning process.

Main Functions in Case Management:
- Clinical Interventions/Discharge and Care Planning Management
- Fiscal Management
- Payer/Referral Management.

Qualifications:
Minimum Requirements:
- Current licensure in Social Work or a clinical or related discipline (RN preferred), in accordance with state guidelines for case management and/or discharge planning.
- Previous experience in Case Management and Discharge Planning is preferred.
- CCM Certification is preferred.
