About the RoleAs a MDS Coordinator, you will play a critical role in ensuring the highest level of care for our residents. Your primary responsibilities will include:
- Managing the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create personalized care plans
- Promoting cross-functional communication to ensure care plans are followed
- Ensuring appropriate reimbursement for resident care services
- Staying up-to-date with current federal, state, and local standards, guidelines, and regulations
We're seeking a highly motivated and experienced MDS Coordinator who possesses:
- A current, valid state RN license
- At least 2 years of MDS experience
- Prior experience in Long-Term Care
