As a member of our team, you will be responsible for delivering exceptional patient experiences, working collaboratively with our healthcare providers, and contributing to the overall success of our practice.
Responsibilities:- Provide phone triage support for patients, ensuring timely and effective communication with our healthcare providers.
- Participate in a relationship-based care approach to deliver personalized patient care.
- Perform Nurse Visits, including Annual Wellness Visits, Advance Care Planning, and Transitional Care Management calls.
- Conduct point-of-care tests, EKGs, and other tests as directed by clinicians.
- Triage walk-in patients and ensure appropriate disposition.
- Respond to telephone messages under the direction of our healthcare providers.
- Function as a liaison between patients, physicians, and staff to provide accurate communication.
- Assure adequate coverage within the unit when needed.
- Assist with staff education and evaluation of clinical competencies for new and existing employees.
- Support the site by acting as the liaison for JCAHO, POCT, Tracer monitor for Safe Practice Behavior Monitoring (SPBM) tools & JCAHO Tracers.
- Administer medications, monitor vital signs, and perform routine treatments and procedures prescribed by our healthcare providers.
- Stay current with policies and procedures and participate in quality improvement activities.
- Participate in special projects.
- A graduate of a state-approved or National League of Nursing accredited School of Nursing.
- Two years' experience as a Registered Nurse, with physician office experience preferred.
