Catholic Charities of the Archdiocese of St Paul and Minneapolis is seeking a compassionate and detail-oriented individual to join our Homeless Elders team as a Social Worker I.
About the RoleThis is an exciting opportunity to make a meaningful difference in the lives of older adults experiencing homelessness. As a Social Worker I, you will work closely with clients to provide person-centered care, case management services, and short-term counseling to help them achieve their goals and access supportive services.
Key Responsibilities- Provide comprehensive case management services, including intake, interview, and assessment, goal setting, and information and referral services.
- Assess clients' needs and provide short-term counseling, skill-building techniques, and aftercare to help them overcome barriers to housing and other challenges.
- Work with clients in a variety of outreach settings, both at agency sites and in the community, to provide client services and support.
- Document and review client files to ensure accuracy, relevance, and timeliness of data.
- Make referrals to other professionals and coordinate joint efforts to address clients' needs and problems.
- Minnesota Board of Social Work Approved License (e.g. LSW, LGSW, LISW, LICSW).
- One (1) year of broad-based clinical experience preferred.
- Experience in individual and group counseling preferred.
- Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
- CPR/First Aid Certification required.
