The Social Services Assistant plays a crucial role in supporting the social service team by delivering administrative assistance throughout the patient journey from admission to discharge.
**Key Responsibilities**
- Engage with patients/residents and their families to offer support and relevant resource information.
- Address concerns raised by patients/residents and their families.
- Assist in the transfer of patients/residents within the facility to ensure a smooth transition and proper adjustment, documenting the process accordingly.
- Contribute to the healthcare decision-making process within the facility under the guidance of a qualified social worker or consultant.
