Healthcare Coordinator RN

Job Summary:
We are seeking a motivated and experienced Registered Nurse to join our team as a Healthcare Coordinator. As a key member of our care coordination team, you will be responsible for providing comprehensive community-based care coordination and management for clients with chronic and complex healthcare needs.
Responsibilities include direct assessment, planning, and collaboration with various service providers to ensure members receive appropriate care in the least restrictive environment. You will work closely with members, caregivers, and healthcare professionals to develop personalized care plans that address their unique needs and goals.
A successful candidate will possess excellent communication and interpersonal skills, with the ability to effectively collaborate with diverse stakeholders. Strong organizational and time-management skills are essential in managing multiple cases simultaneously.
We offer a competitive salary of $65,000 - $80,000 per year, depending on experience, plus benefits including health insurance, retirement plan, and paid time off. Key Responsibilities:
- Provide comprehensive community-based care coordination and management for clients with chronic and complex healthcare needs
- Conduct direct assessments, develop personalized care plans, and coordinate with service providers to ensure timely and effective care delivery
- Collaborate with members, caregivers, and healthcare professionals to address social determinants of health and promote optimal health outcomes
- Manage multiple cases simultaneously, prioritizing tasks and meeting deadlines while maintaining accurate records and documentation
- Communicate effectively with diverse stakeholders, including members, caregivers, and healthcare professionals
Requirements:
- Current Registered Nurse license in the state of Delaware
- 2+ years of case management and discharge planning experience, including experience discharging members from a facility setting
- Strong communication and interpersonal skills, with the ability to effectively collaborate with diverse stakeholders
- Excellent organizational and time-management skills, with the ability to manage multiple cases simultaneously
- Ability to travel throughout the state, with occasional evening or weekend hours required
