Job Summary

We are seeking a highly motivated and professional individual to support the care management discharge planning process in our Inpatient Department. As a key member of our team, you will be responsible for completing various tasks, including obtaining and recording authorizations, facilitating referrals, and expediting discharges.

Responsibilities
  • Obtain and record authorizations for ancillary services and denial reporting.
  • Facilitate, identify, and document all referrals, along with patient choice, to appropriate facilities or agencies.
  • Expedite discharges by transmitting appropriate documentation to providers for acceptance of patient.
  • Document all interactions with payers and communicate status with appropriate staff.
  • Communicate status with case management staff and arrange for patient transfer.
  • Function as a key point of contact between case management staff, admissions, and payers.
Requirements
  • High School diploma equivalency or 1 year of applicable cumulative job-specific experience required.
  • Required professional licensure/certification can be used in lieu of education or experience, if applicable.
Additional Preferences
  • Obtains and records denials received from payers and manages authorizations, including uploading to drive.
  • Sets up Peer to Peer calls for the Physician Advisor.
  • Documents all interactions with payers and communicates status with the appropriate staff.
  • Assists the Care Management department with delivery of CMS forms, making PCP follow-up appointments, and assists the unit care managers with certain care management functions, such as uploading clinical information and making referrals as needed.
  • Performs secretarial functions, such as telephone coverage for the department, department assignment entry, faxing discharge planning orders to the appropriate departments.
  • Participates in orientation activities, such as managing initial orientation and education of ancillary staff as needed.
