Job Summary:

We are looking for a compassionate and dedicated nursing professional to join our team. As an LPN/RN, you will be responsible for providing high-quality patient care, assessments, and medication administration in a home health setting.

Responsibilities:
  • Assess and chart patient conditions at each visit
  • Complete evaluation tasks, including medication and vital signs
  • Dress or redress wounds and assess healing progress
  • Provide education to patients and families on proper home health care procedures and strategies
  • Coordinate with physicians, PT, OT, and other individuals in the patient's care plan
Requirements:
  • PA LPN or RN License must be active and valid
  • Driver License
  • Ability to pass a background check and drug screening
  • Ability to operate a computer and use an Electronic Medical Record system
  • Ability to utilize electronic visit verification system via a mobile app
