Job Summary:

As a Public Health AmeriCorps Community HEART Work Member II, you will play a vital role in improving the health and well-being of individuals and communities. This position requires a strong understanding of community health principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities:

• Provide screening and referral services to connect individuals and families to social and medical services
• Develop and implement care plans to address individual barriers and goals
• Act as an intermediary between individuals and clinical and community service providers
• Educate and inform individuals about health, wellness, and social services
• Collect and maintain data on referrals and outcomes

Requirements:

• High School Diploma or equivalent
• Bilingual (English/Spanish) preferred
• 2 or more years of experience working in the community or healthcare field
• Promotor(a) or Community Health Workers Certification or willingness to obtain within six months of start
