Job Responsibilities- Provide compassionate nursing care to residents in post-acute facilities
- Monitor residents' physical and mental conditions and track vital signs
- Perform resident intakes, document medical histories, and track health concerns
- Administer and verify medications, track use and effects
- Cultivate a comfortable and safe facility for residents
- Track medical records and charts
- Assist with IVs, immunizations, and phlebotomy procedures
- Manage medical supplies and maintain inventory
- Follow and adhere to resident care plans
- Work with staff to orient and train new colleagues
- Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
- Completion of approved LPN educational program
- High school graduation, GED, or equivalent
- At least 6 months of experience (LPN or similar role)
- Ability to lift a minimum of 50 lbs
