Job Summary

We are seeking a dedicated and compassionate Registered Nurse to provide high-quality medical care to patients in their homes. As a Home Health RN, you will play a vital role in delivering essential medical services, promoting patient well-being, and supporting their recovery.

Key Responsibilities
  1. Patient Assessment and Care
  • Conduct thorough patient assessments to develop and implement individualized care plans.
  • Administer medications, wound care, and specialized treatments as prescribed by physicians.
Monitoring and Education
  • Monitor patient conditions, provide education, and support patients and families in managing illnesses and recovery.
  • Offer guidance on healthy practices, medication management, and disease prevention.
Collaboration and Documentation
  • Coordinate care with the interdisciplinary team to ensure holistic patient well-being and meticulously document patient progress and care provided in the EMR system.
  • Maintain accurate and timely documentation to ensure comprehensive patient records.
Emotional Support
  • Provide emotional support and empathy during challenging periods of illness and recovery.
  • Advocate for patients' needs and preferences, ensuring their well-being and comfort.
Adaptability and Problem-Solving
  • Adjust care plans as needed, applying critical thinking and problem-solving abilities in varied home settings.
  • Address unexpected situations or emergencies effectively and efficiently.
Requirements
  1. THESE ARE THE PRIMARY REQUIREMENTS YOU MUST MEET TO BE CONSIDERED: - WOUND VAC EXPERIENCE, PICC LINE EXPERIENCE, OSTOMY CARE EXPERIENCE, AND CATHETER CARE EXPERIENCE
  2. Education: Registered Nurse (RN) license in good standing and completion of a nursing program from an accredited institution.
  3. Experience: Prior experience in home health or a related field is preferred.
  4. Compassion: Demonstrated empathy, sensitivity, and a patient-centered approach to care.
  5. Communication Skills: Excellent verbal and written communication for effective patient and family interaction.
  6. Adaptability: Ability to work independently and adapt to different home environments while ensuring patient safety.
  7. Ethical Conduct: High ethical standards, maintaining patient confidentiality, and respecting their autonomy.
  8. Technical Skills: Proficiency in utilizing Electronic Medical Records (EMR) and other healthcare software.
  9. Driver's License: Some roles may require a valid driver's license and reliable transportation for home visits.
