Job Summary:

We are seeking a Licensed Practical Nurse to provide high-quality patient care in a home health setting. As a member of our healthcare team, you will be responsible for delivering compassionate and individualized care to patients in their homes. You will work closely with our interdisciplinary team to develop and implement patient care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Collaborate with our interdisciplinary team to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure that patient needs are met and that care is delivered in a timely and efficient manner.
  • Participate in ongoing education and training to stay current with best practices in home health care and to enhance your skills and knowledge as a Licensed Practical Nurse.
Requirements:
  • Graduation from a Licensed Practical Nursing program approved by the State Board of Nursing.
  • Current licensure as a Licensed Practical Nurse in the state where you will be practicing.
  • Basic Life Support (BLS) certification.
  • Ability to work independently and as part of a team to provide high-quality patient care.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
