We are seeking a highly skilled and experienced MDS Coordinator to join our team at Sheffield Manor Nursing and Rehab. As a key member of our clinical operations team, you will play a critical role in ensuring the highest quality of care for our residents.
Key Responsibilities- Conduct thorough and accurate resident assessments, utilizing the Resident Assessment Instrument (RAI) process to identify care needs and develop effective care plans.
- Collaborate with the interdisciplinary team to coordinate care planning, ensuring seamless transitions and optimal resident outcomes.
- Supervise and mentor the Care Management Nurse and MDS Nurse, providing guidance and support to ensure successful completion of RAI assessments and care plans.
- Stay up-to-date with American Association of Nursing Assessment Coordinators (AANAC) requirements and best practices, ensuring compliance and excellence in MDS coordination.
- Participate in PPS meetings and maintain accurate records of Medicare days and RUGs utilization.
- Registered Nurse (RN) licensure
- AANC certification or RAC-CT preferred
- Experience as an MDS Nurse or in a related role
- Knowledge of the RAI process and Prospective Payment Process (PPS)
