As a

Hospice Care Nurse

(LPN), you will provide essential support during critical times.

Key Responsibilities:
As a Hospice Care Nurse (LPN), your duties will include:
- Presenting the Home Health/Hospice benefits to patients and their families or representatives
- Assessing signs and symptoms of discomfort, including pain, and taking prompt, appropriate actions
- Following the established care plan while collaborating with the RN Case Manager and the Interdisciplinary Team, including physicians as necessary
- Communicating with patients' families or friends to provide guidance, comfort, and discharge instructions.

Qualifications:
To be considered for the Hospice Care Nurse (LPN) position, you will need:
Licensure:
- A current license to practice as a Licensed Practical Nurse (LPN) in the relevant state
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to and from care sites
- A valid state-issued driver's license and automobile insurance; willingness to travel
- Preferred one (1) year of experience as a Licensed Practical Nurse (LPN) in a hospice or similar environment
- Hospice bedside and continuous care experience is preferred.
