Your Role:
As a Hospice Care Licensed Practical Nurse (LPN), your responsibilities will include:
- Presenting the Home Health/Hospice benefits to patients and their families
- Assessing discomfort signs and symptoms, including pain, and taking appropriate actions
- Following the established care plan while collaborating with the RN Case Manager and the Interdisciplinary Team, including physicians
- Communicating effectively with patients' families to provide guidance, comfort, and discharge instructions.
Qualifications:
- Current licensure as a Licensed Practical Nurse (LPN) in the relevant state
- Valid CPR/AED/BLS/First Aid certification
- Reliable transportation to and from care sites
- Valid state-issued driver's license and automobile insurance
- One (1) year of preferred experience as a Licensed Practical Nurse (LPN) in a hospice or similar environment
- Preferred experience in hospice bedside and continuous care.
