Overview

Amedisys is a leading provider of home health, hospice, and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system. We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team.

Responsibilities
  • Perform patient comprehensive assessments and collaborate with the patient's multidisciplinary care team to develop and implement an individualized plan of care.
  • Facilitate interdisciplinary communication and follow-up through care coordination on assigned patients.
  • Make referrals to other disciplines as indicated by the patient's needs or document the rationale for not doing so.
  • Participate in clinical outcomes monitoring, follow-up, and care center performance improvement initiatives.
  • Initiate, plan, and provide staff education and perform competency and skills validation for staff as needed.
Qualifications
  • One year of nursing experience.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
