Perinatal Nurse Navigator

Mission Hospital is seeking a skilled Perinatal Nurse Navigator to join our team. As a key member of our healthcare team, you will play a vital role in supporting patients through complex healthcare environments, optimizing care and outcomes.
Key Responsibilities- Facilitate patient follow-up through coaching, information transfer, and communication to improve continuity of care and enhance medical management of complex diseases.
- Guide patients and families through the healthcare system, assisting with access issues to expedite timeliness of care and treatment.
- Build relationships with patients and physicians, coordinate the defined plan of care, assist with appointments, resolve barriers, provide education, and connect with community resources.
- Manage resources to achieve efficient, high-quality outcomes for patients, including support with interdisciplinary and cross-facility coverage and collaboration.
- Collect and track data to promote navigation program performance in line with HCA Healthcare, division, and facility goals.
- Basic Cardiac Life Support (BLS)
- Service Line Specialty Certification – Preferred
- One to three years of clinical experience required
- Three plus years of experience as a nurse navigator and/or patient educator in service line – Preferred
- Required Education: Bachelor of Science in Nursing or enroll in a BSN or advanced degree program and obtain a BSN within 5 years of hire date into RN position
- Required License/Certifications: Must have and maintain current licensure as a registered nurse with the North Carolina Board of Nursing
