Job Summary

Mission Hospital is seeking a skilled Perinatal Nurse Navigator to join our team. As a key member of our healthcare team, you will play a vital role in supporting patients through complex healthcare environments, optimizing care and outcomes.

Responsibilities
  • Facilitate patient follow-up through coaching, information transfer, and communication to improve continuity of care and enhance medical management of complex diseases.
  • Guide patients and families through the healthcare system by assisting with access issues, coordinating the defined plan of care, and connecting with community resources.
  • Assume responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for patients.
  • Collect and analyze data to track navigation program performance and identify opportunities to streamline care practice through evidence-based guidelines.
Requirements
  • Basic Cardiac Life Support (BLS) certification
  • Service Line Specialty Certification (preferred)
  • One to three years of clinical experience required
  • Three plus years of experience as a nurse navigator and/or patient educator in service line (preferred)
  • Required Education: Bachelor of Science in Nursing or enroll in a BSN or advanced degree program and obtain a BSN within 5 years of hire date into RN position
  • Required License/Certifications: Must have and maintain current licensure as a registered nurse with the North Carolina Board of Nursing
