Job Overview

We are seeking a highly motivated and compassionate Community Outreach and Support Coordinator to join our team at TELACU Residential Management, Inc. (TRM). As a key member of our staff, you will play a vital role in providing exceptional support services to our residents, ensuring their comfort, well-being, and satisfaction with their living experience.

About the Role

The Community Outreach and Support Coordinator is responsible for serving as a liaison between TRM and its residents, families, and community partners. This position requires excellent communication, problem-solving, and organizational skills, as well as a deep understanding of the needs and challenges faced by low-income older adults and individuals with disabilities.

Responsibilities
  • Serve as resident advocate, addressing daily life issues and well-being concerns in a timely and professional manner.
  • Initiate and manage the 504 process for reasonable accommodations, working closely with maintenance staff and residents to ensure seamless completion.
  • Develop and implement individualized Supportive Services Plans, incorporating evidence-based programming and services to facilitate safe aging in place.
  • Provide case management, assessing resident needs, updating plans, and making referrals to service providers as necessary.
  • Collaborate with property management teams on outreach and education efforts, creating resource directories and promoting community programs.
  • Maintain accurate records and reports using AASC Online, adhering to confidentiality and data protection policies.
Requirements

To succeed in this role, you will need:

  • A Bachelor's degree in Social Work, Gerontology, Psychology, or Counseling, or an equivalent combination of education and experience.
  • Bilingual English/Spanish proficiency preferred.
  • Strong advocacy, problem-solving, and communication skills.
  • Ability to work independently and as part of a team, prioritizing tasks in a fast-paced environment.
  • Familiarity with program services supporting elderly and special needs populations.
  • Cultural competency and sensitivity when interacting with diverse populations.
