Job Summary

We are seeking a highly motivated and compassionate Peer Navigator to join our team at Healthcare in Action. As a Peer Navigator, you will play a critical role in supporting individuals experiencing homelessness in the Long Beach, CA area. Your primary responsibility will be to provide outreach and support services to help members manage their health and improve their overall well-being.

Key Responsibilities
  • Build strong relationships with members by sharing your personal experiences and providing guidance on healthy behaviors.
  • Coach members to develop strategies that minimize risks associated with common health conditions and behaviors, and connect them to resources that address barriers to care.
  • Link members to local, county, and state services and resources, and follow up with them to ensure they are receiving the support they need.
  • Conduct regular outreach to patients at the Navigation Center and in the community, providing support for medical needs and accompanying members to medical appointments.
  • Collaborate with staff at the Navigation Center and Healthcare in Action team members to ensure comprehensive care management for patients.
  • Introduce tools and strategies to promote self-management and self-efficacy, and complete all required documentation in a timely fashion.
Requirements
  • G.E.D. or high school diploma, plus 3+ years of experience working with patients experiencing homelessness and mental health conditions.
  • Bachelor's degree preferred, plus 1-2 years of experience in a similar role.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Reliable transportation and a valid driver's license, with a clean driving record.
  • Proficiency in operating a computer and navigating multiple systems, including electronic health records and care management software.
