About the Position

We are seeking a skilled and compassionate Nurse Practitioner to join our team as a PRN/Part Time provider. In this role, you will conduct home risk assessments, medical history reviews, and provide health education to individuals in their homes. You will also work collaboratively with our team of clinicians, case managers, and social workers to develop and implement care plans that meet the unique needs of each individual.

As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work in a variety of settings, including home visits, skilled facility visits, and telehealth visits. You will also have access to our comprehensive training program, which will equip you with the skills and knowledge you need to succeed in this role.

Responsibilities
  • Conduct comprehensive health assessments, including medical history reviews and risk factor identification
  • Develop and implement personalized care plans to help individuals achieve better health outcomes
  • Collaborate with our team of clinicians, case managers, and social workers to ensure seamless care coordination
  • Provide health education and support to individuals and their families
  • Work in a variety of settings, including home visits, skilled facility visits, and telehealth visits
Qualifications
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
Travel Requirements

We require our Nurse Practitioners to have a valid state driver's license and be able to drive a car. You will also need to have proof of adequate automobile insurance coverage for the state of residence. Travel is required to meet patients where they live, and you will need to be comfortable and flexible with frequent change.
