About the Job

The Licensed Practical Nurse (LPN) is a vital member of our team at Washington Square Nursing & Rehab, responsible for providing direct and indirect nursing care to patients ranging from young adults to the geriatric population, who may be experiencing medically complex conditions or in need of rehabilitation and wound care.

This role involves monitoring patient conditions and reporting any changes to the Registered Nurse (RN), as well as executing the nursing process effectively when caring for patients. Collaboration with team members across different departments while demonstrating our mission and core values is integral to this role.

Key Responsibilities:
  • Supervise Certified Nurses Aides under the direction of the RN.
  • Collect and monitor patient care data based on observation and history.
  • Contribute to the development of care plans and discharge plans.
  • Implement patient care according to the care plan, medical orders, and in collaboration with the healthcare team.
  • Assist in evaluating patient care outcomes and revise care plans based on changing patient needs.
  • Participate in the training of new personnel and students.
  • Document health care delivery and the nursing process in adherence with unit standards and policies.
  • Prioritize tasks and responsibilities effectively to complete duties within assigned time frames.
  • Understand the rationale behind medications and treatments and administer them correctly.
