About the Role:

We are seeking a compassionate and skilled End-of-Life Care Specialist to join our team at Affinity Care of Ohio. As a hospice aide, you will work under the supervision of a registered nurse to provide care and support to patients with life-limiting illnesses.

Job Description:

The End-of-Life Care Specialist will be responsible for performing various tasks as assigned in accordance with the patient's plan of care to meet their personal needs and promote comfort. This includes providing personal care such as bathing, grooming, and dressing, as well as assisting with feeding, ambulation, and exercise.

The ideal candidate will have a strong understanding of hospice philosophy and be comfortable working with terminally ill patients. They will also possess excellent communication skills, both written and verbal, and be able to document care given in electronic medical records.

Requirements:

  • Completion of a state-approved certified nursing assistant course or equivalent training
  • Current Healthcare Provider CPR certification
  • Covid vaccination required due to nature of job
  • High school diploma or General Equivalency Degree (GED)
  • Ability to read and follow written instructions and document care given
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill
  • Prior experience in hospice, orderly, acute, rehabilitative, or long-term care facility preferred
  • Basic computer skills to document in electronic medical records (EMR)
  • At least 18 years of age
  • Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order

Estimated Salary Range: $40,000 - $60,000 per year
