Transplant Licensed Clinical Social Worker

As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and their families undergoing transplant procedures.
Responsibilities- Collaborate with medical doctors and other healthcare professionals to identify and address significant social and emotional factors impacting patient well-being.
- Develop and implement individualized treatment plans to meet the unique needs of each patient and their families.
- Provide therapy activities for individual, group, and couples counseling to promote emotional healing and support.
- Assist in the development of departmental policies and procedures to ensure high-quality patient care.
- Provide practicing oversight of Certified Social Workers, Social Service Workers, and interns in accordance with Utah state law.
- May perform Case Management duties to ensure seamless care coordination.
- Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
- Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system.
- Experience with conflict resolution and counseling individuals, couples, and groups.
- Medical social work experience, preferably in a hospital setting, with a focus on transplant care.
- Two years of directly related experience in transplant care.
- Experience working with community resources, instructing, and supervising Social Workers, Interns, and other caseworkers.
This position involves intermediate work that may exert up to 50 pounds and may consistently require lifting, carrying, pushing, pulling, or otherwise moving objects while providing patient care to those requiring psychiatric care.
