We are seeking a highly skilled Licensed Social Worker to join our interdisciplinary team at Heartland Hospice. As a key member of our team, you will provide essential psychosocial support to patients and their families, utilizing your expertise to identify needs, goals, and interventions.
Responsibilities- Conduct comprehensive psychosocial assessments to identify patient and family needs
- Develop and implement individualized plans of care to address identified needs
- Provide emotional support and counseling to patients and their families
- Collaborate with the interdisciplinary team to ensure comprehensive care
- Document patient progress and outcomes
- Master's Degree in Social Work from a CSWE-accredited school
- Licensure as required by the state in which the hospice is located
- At least one year of experience as a Social Worker in a healthcare and/or hospice setting
- Knowledge of terminally ill patients and their families, as well as understanding of hospice and psychosocial dynamics of illness, loss, and death
