Job Description

The Licensed Practical Nurse (LPN) - Clinic provides support to providers in the delivery of timely, quality patient health care services in a clinic environment. Some duties may include collecting patient information and data, providing patient care, communicating with patients, and collaborating with care team members.

Individuals in this role will work in an outpatient clinic setting, providing patient care support as delegated by the Provider or RN. This includes administering therapies, scheduling follow-up visits, and providing patient care pre/post encounter or procedure.

The LPN will also collaborate with the Provider and team on improving and managing quality goals, and may start, monitor, and discontinue IVs (dependent on site needs and training). Additionally, the LPN will room patients, manage patient flow, and obtain adequate patient medical information/data.

The LPN will perform rooming and accurately measure patients' vital signs and other measurement/indicators as necessary and records in the medical record. They will also coordinate communication of medical information and confirm information with patients when necessary and document per practice policy and standards.

The LPN will assist patients, as necessary, to prepare for physician exam, including providing psycho-emotional support. They will also assist in the collection, labeling, and processing of specimens timely/accurately, and assist physician with procedures including preparation of medication as required/needed.

The LPN will organize patient flow so physician can move efficiently from one room to the next, and provide outbound calls to patients to follow up as directed which may include notifying patients of test results. They will also accurately and completely chart information in the patient medical record.

The LPN may checkout and schedule follow-up visits with provider or appropriate staff before patient leaves, and receive or initiate patient phone calls. They will answer phones cordially and professionally in a timely manner, and may confirm or reiterate patient symptom using patient's own words.

The LPN will arrange for appropriate appointment or forward to RN or provider for review/recommendation, and documents all encounters and patient interactions accurately and completely in the electronic medical record.

The LPN will process physician orders, perform any clinical functions within scope of license as ordered by physician, and respond promptly to patient needs and requests. They will review patient charts before the visit to ensure that test results, therapy reports, etc. are available, and provide appropriate instructions to patients as directed by physician.

The LPN will clean, prepare, organize, and re-stock exam rooms, and ensure that required teaching supplies are restocked and readily available. They will also stock and supply exam rooms according to standards and patient needs.
