About the Post Acute Unit
The Post Acute Unit at Helen Porter Rehabilitation & Nursing is a specialized care unit that provides supportive, goal-directed care to patients following surgery and/or hospitalization. Our team of healthcare professionals work together to provide patients with the preparation, support, and education they need to return home as quickly as possible.
Responsibilities
- Provide direct patient care and support to patients with a variety of medical conditions, including strokes, neurological conditions, orthopedic care, respiratory conditions, and cardiac and post-trauma situations.
- Collaborate with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct routine therapy services, including walking, exercises, and supervision of other activities, as needed.
- Provide education and support to patients and their families to promote independence and self-care.
Requirements
- Graduate of an accredited school of nursing.
- Current RN licensure or compact licensure recognized by the State of Vermont required.
- Gerontological nursing and knowledge of supervision techniques preferred.
- Able to read, write, and speak the English language.
- Demonstrate competence as a nursing practitioner.
- BLS certification.
