AHRC NYC is seeking a dedicated and compassionate Treatment Coordinator/QIDP to join our team at our Queens ICF residence. This is a full-time position with a competitive salary range of $55-$65K, plus a comprehensive benefits package.
Job Summary:The Treatment Coordinator/QIDP will play a vital role in coordinating the training and habilitation program of assigned residents, working closely with residential staff, educational and vocational rehabilitation programs, and other needed services.
Key Responsibilities:- Collaborate with interdisciplinary teams to plan, implement, and evaluate individual treatment plans.
- Generate Needs/Capabilities lists annually, with updates as needed, ensuring each assigned client has an activity schedule, ongoing continuous active treatment, and implementation of an activity schedule as documented.
- Act as resident advocate, ensuring cleanliness, adequate clothing, and personalized living spaces.
- Complete reviews of individual treatment plans, summarizing progress or need for program modification based on concrete, measurable data.
- Coordinate services between day and residential components, ensuring timely receipt and filing of IPPs and day program progress notes.
- Train direct care/clinical staff in behavioral interventions and active treatment areas, ensuring all staff can carry out treatment plans as written.
- Coordinate Annual Review process, ensuring reports from all disciplines are collected in a timely manner, preparing annual reports, and generating goals, objectives, and interventions for indicated needs.
Requirements:
• Strong communication and interpersonal skills
• Ability to work collaboratively with interdisciplinary teams
• Experience in residential program coordination or a related field
• Strong organizational and time management skills
• Ability to maintain confidentiality and handle sensitive information
