Core Responsibilities:
- Conduct patient evaluations and formulate individualized treatment plans
- Regularly assess the effectiveness of treatment strategies, participate in team meetings, and engage in family discussions
- Execute discharge evaluations, offer recommendations for home and community safety, and provide necessary referrals and educational resources
- Communicate effectively with the treatment team, physicians, patients, and their families regarding progress and needs
- Collaborate with other healthcare professionals to ensure coordinated care and enhance patient outcomes
Required Qualifications:
Essential Skills and Experience:
- Bachelor’s degree in Physical Therapy from an accredited institution
- Valid and unrestricted Physical Therapist license in the practicing state
- Current Basic Life Support or Cardiopulmonary Resuscitation certification as per state regulations
- Preferred: Minimum of one year of experience in physical therapy
- Knowledge of rehabilitation techniques for complex neurological conditions
- Strong verbal and written communication skills
- Ability to lift up to 50 pounds; may involve lifting patients up to 100 pounds while adhering to safety protocols
- Physical requirements include fine motor skills, visual acuity, and the ability to stand or walk for extended periods
- Additional physical demands include frequent pushing, pulling, bending, stooping, reaching, kneeling, and positioning
