Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Woodbridge, Virginia.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence and improve the overall health and well-being of our clients.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Implement evidence-based interventions to improve clients' mobility, strength, and balance.
  • Collaborate with our team to ensure seamless care coordination and communication.
  • Provide education and training to clients and their families on rehabilitative care and activities.
  • Document client progress and outcomes in a timely and accurate manner.
Requirements:
  • Current VA Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
