Job Description: As a Patient Navigation Specialist at Henry Street Settlement, you will be part of a dedicated team working towards improving the lives of our patients. In this role, you will collaborate with care teams to maintain program alignment and ensure patient care continuity. You will also attend all MCSP staff and contractor meetings, and support ad hoc contractor activities consistent with the Scope of Work (SOW). Additional responsibilities include performing duties as assigned by the Hospital Program Manager and maintaining a professional demeanor when representing the agency in meetings with funders/program partners. Estimated salary range: $60,000 - $80,000 per year.
