Patient Care Coordinator

UnitedHealth Group is seeking a highly skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a critical role in ensuring the smooth operation of our healthcare services. Your primary responsibility will be to coordinate patient care activities, including scheduling appointments, managing medical records, and communicating with patients and healthcare providers.

- Communicate effectively with patients and healthcare providers to ensure seamless care coordination.
- Manage and maintain accurate and up-to-date patient records.
- Assist healthcare providers with distribution of medical results and other patient-related information.
- Provide exceptional customer service to patients and their families.
- Collaborate with internal teams to ensure efficient and effective care coordination.
- Perform other related duties as assigned.
- High School Diploma or equivalent required.
- 1+ years of customer service experience preferred.
- 1+ years of healthcare experience preferred.
- Excellent communication and interpersonal skills.
- Ability to multitask and prioritize tasks effectively.
- Basic computer skills and proficiency in Microsoft Office.
